2022 Yearly Simplified Group Instruction
Registration Guidelines
To secure enrolment in desired program/series/class time, complete the following :
1. Click on the button directly below for all information on Fall/Winter Junior & Adult Programs.
2. Text Coach Chuck Lynch (Director of Instruction@ 847-436-1652) with your Name, Parents name (if applies), Email, Phone, Students name & Age (if applies) , & desired program/series/class time that you are wanting to register for. Coach Chuck will let you know if there is space in the program.
3. Download registration form directly below. Fill out, sign, add credit card information to secure registration. Your credit card
WILL NOT be charged if paying by cash or check. (See #5) This is only to guarantee your placement in the program/series.
50% of the series fee will be charged for cancellations made within 7 days of program start date.
4. Return registration form before the first day of class by any of the following methods:
- Scan or take a picture of the completed form and email to [email protected].
- Put completed registration form in a sealed envelope labeled "PBG Registration" and come to the Addison Links and Tees Driving Range building (hand to range attendant) OR inside the Golf Dome (drop in mail slot in front of bay 19.) NO ENVELOPE CASH PAYMENTS!
- Mail directly to Coach Chuck Lynch at 467 E. Providence Rd, Palatine, IL, 60074. Do not include cash payments in mail.
Without Completed Registration Form, Your Spot WILL NOT be held.
5. Arrive to first class 15 minutes early to complete registration and make full payment of $300 by either (cash, check-payable to Play Better Golf Inc., or Credit Card (+3% convenience fee.) There will also be a brief parent meeting if applicable.
